Dear Band and Community Members,

I am excited to announce a Community Health Needs Assessment for the Lac Des Mille Lacs First Nation in conjunction with Dr. Cheryl Everall of “The Joseph Esquega Health Center”.

This study investigates the impact of legal and illegal substances such as tobacco, alcohol, opiates and prescription medication on Lac Des Mille Lacs First Nation band and community members.

More importantly, as we move forward in rebuilding our community, this health care assessment has also been designed to help identify problematic areas and assess our needs to implement community based programs. By participating in this survey, together we can be proactive in creating a healthy tomorrow.

 

During the community health fair, I was able to introduce myself and present our community health assessment survey. I collected 20 responses! Chi Miigwetch to those whom participated; your response is very much appreciated and valuable to this research.

The survey can be submitted via mail or electronically via https://www.surveymonkey.com/s/S6W6Z5B.  It will be distributed to those members living in Thunder Bay and surrounding regions however all members are welcome to take the survey online. You must be at least 16 years of age and a LDMLFN band or community member to participate in this survey. Participants are eligible to win one of four $100 dollar gift cards; drawings will be held starting June 1, 2015.

Your participation is anonymous and all information will be kept strictly confidential.

This study is fully supported by the Chief and Council.
