STATUS CARDS – INDIAN REGISTRATION ADMINISTRATOR

Congratulations go out to Honey Chicago who has successfully completed the necessary training as our Indian Registration Administrator. The training authorizes Honey to administer laminated Certificate of Indian Status cards (CIS) only and not the Secure Certificate of Indian Status (SCIS).  The new SCIS will only be issued at AANDC offices.  If members have a new SCIS card, they will not be given a laminated card as once you have a new one, you cannot revert back to the old laminated ones.  Members may contact AANDC at 807-623-3534 to get information regarding the new SCIS cards.  Honey is available to issue Certificate of Indian Status cards for our members (by appointment only) on Wednesday afternoons from 1:00 p.m. to 3:00 p.m. 

Honey will also be updating events such as: Registering a Child (Birth) or Reporting: Death, Marriages, Divorces, Reverting to Maiden Name, Amendments.

ID Requirements for CIS Cards:

2 pieces of ID or 1 piece of ID and 1 guarantor form. If they have NO ID they will have to apply online to get a birth certificate by clicking here.

Acceptable ID includes:                                              

  • Valid Driver’s license                                      
  • Valid Health Card                                                        
  • Birth certificate (Long or Short)                                               
  • Valid Fire Arms license                                               
  • Valid Student ID card
  • Valid Nexus Card (counts as 2 pieces)
  • Valid Passport (counts as 2 pieces)

16 & Older

  • Must provide 2 pieces of ID
  • Or 1 piece of ID and a Guarantor Declaration Form

15 & Younger

  • Must have 1 piece of ID of PARENT or LEGAL GAURDIAN and 1 piece of ID for CHILD
  • Or 1 piece of ID of PARENT or LEGAL GUARDIAN and 1 GUARANTOR DECLARATION form

(IMPORTANT: IF THE LEGAL GUARDIAN IS APPLYING ON BEHALF OF THE CHILD, A COPY OF THE LEGAL GUARDIANSHIP ORDER NAMING THE GUARDIAN IS MANDITORY.)

  • CHIEF/ COUNCILLOR/ INDIAN REGISTRATION ADMINISTRATOR
  • DENTIST/ MEDICAL DOCTOR/ CHIROPRACTOR/JUDGE/ MAGISTRATE/ POLICE OFFICER
  • LAWYER OR NOTARY PUBLIC
  • MINISTER (AUTHORIZED TO PERFORM MARRIAGES)
  • PRINCIPAL
  • PHARMACIST
  • PROFESSIONAL ACCOUNTANT/ SOCIAL WORKER
